Roster Sticker Template

USA Hockey & Massachusetts Hockey require all teams to use pre-printed roster labels on all 3 copies of the scoresheet.

Each label must contain:

1. Massachusetts Hockey Code - MAH Code--MAHXXXX-XXX-- found on the official roster submitted to USA Hockey.
2. Team Name - This is how your team is listed in the VHL (Example: Methuen Rangers Squirt 1)
3. All rostered - Players must be listed either ALPHABETICALLY or NUMERICALLY. The template has spots for 20 players; the maximum allowed by USA Hockey.
4. All rostered coaches entered in the following format: First Name, Last Name, CEP# Level, and expiration year. The template has spots for 4 coaches; the maximum allowed by USA Hockey.
5. The names of any players that will be playing up or coaches who will be on the bench will need to be handwritten onto each roster sticker.

The template is designed to work with any 2"x4" blank label, such as Avery Labels 5263.
